Merging of computer systems taking place between 4/5/18 to 8/5/18
London Medical Practice will be unable to take any new registrations between Fri 19/4/18 to Mon 15/5/18 due to the GP links being closed down in preparation for the computer system merge.
We can start taking on new registrations again from Tuesday 15th May 2018.
In the mean time, patients wishing to register and requiring medication urgently or needing to see a doctor, we can register you as a temporary resident and deal with any urgent matters. However you will be required to come back and register properly from Tuesday 15th May 2018.
NO ACCESS TO MEDICAL RECORDS
The process for merging the computer systems will actually take place between Fri 4/5/18 from 1pm, over the weekend until Tues 8/5/18. During the merging process, in the above time period, we will have no access to your medical records.
- We will be unable to safely issue any of your regular medication. In an absolute emergency, you could go to your usual pharmacist and obtain a small emergency supply but likely you will have to pay a small charge for this.
- Please do not call the surgery unless you have an urgent problem or an emergency after 1pm on Fri 4/5/18.
If you call on Tues 8/5/18 before we regain access to your medical records, you may be asked to call back again.
Important notice for patients
On Friday 27th April 2018 our EPS is being switched off due to the upcoming merge of the computer systems with the other surgeries. This means that we will not be able to receive requests online or be able to send prescriptions electronically; this means that we are only able to print prescriptions. Please inform your usual pharmacy if you will need them to collect your prescription from the surgery for you.
This will be the case until the afternoon of Tuesday 8th May 2018 once all of the systems have been set up.

We would like to announce some changes we are proposing in order to create a stronger, more resilient Practice and secure Primary Care services for our patients into the future.
This week, we are making our patients aware of the proposal to merge with Barnwood, Heathville, London and Saintbridge Medical Practices on the 1 April 2018.
With the growing complexity within Primary Care and a rising population it is becoming increasingly difficult to manage a small Practice and offer a good level of care to patients.
We believe a merger would create a more resilient and sustainable Practice with the resources and expertise needed to manage all the demands of General Practice, both administratively and clinically.
It would also increase our ability to forge closer and better co-ordinated partnerships with a range of providers of community services and support for the benefit of local people.
Proposed Practice Merger
Over the last few months, we have been talking to the GPs and staff from Barnwood, Heathville, London and Saintbridge Medical Practices in Gloucester about merging our four practices together.
During these discussions it became clear that all the surgeries shared a similar approach to patient care.
After careful consideration, we have reached the conclusion that a merger would be a very positive development and believe it would not only secure the future of the Practice, but also enable us to offer a better service to all our patients.
What this Means to You
This does not mean that any anyone registered with us would be removed from our patient list.
The same Doctors, Practice Nurses and Staff would still be part of the new team. You would be able to see your chosen Doctor as usual.
The principles of quality and personal clinical care would continue to be of uppermost importance to the new Practice and our aim would be to enhance services and flexibility whilst ensuring stability for our patients. An enlarged Practice will also allow us to improve access to services.
The proposed new, combined Practice would have one patient list and a new name – we would be very happy to receive your suggestions for this.
The Practices are enthusiastic about the proposed new Partnership. We would work closely over the coming months to integrate systems and combine processes. Our main priority would be to ensure a seamless transition for our patients and minimise any disruption.
We see this as an opportunity for us to build a secure future and a Practice better able to manage the challenges of the NHS and offer the best for our patients with excellent resources.
There would be a new combined practice boundary drawn to include all the areas currently covered by the 4 Practices. You will be able to attend the surgery building which best suits you.

In summary, we believe the merger would:
enable us to offer a greater range of services to patients
enable us to provide greater choice of appointments
create a wider range of skills and expertise in the Practice, with more secure staffing levels
help us to make better use of staff time
enable us to use our resources more flexibly to meet patient demand.

Electronic Prescribing Service
London Medical Practice is one of the first practices in the UK to introduce an Electronic Prescription Service. This service will mean that your GP will be able to send your prescription electronically to the Pharmacy you choose to get your medication from – without the need for paper in most cases. The Electronic Prescription Service is reliable, secure and confidential.
If you would like to use the Electronic Prescription Service:
- You will need to ‘nominate’ a pharmacy to receive your prescriptions electronically. You can ask a member of staff in the pharmacy to arrange this, or a member of staff in the surgery
- You can then continue to order your prescriptions in the usual way, but instead of a paper prescription being collected by you or your pharmacy, the prescription will be sent to them electronically.
- You should try to ensure that on most occasions you collect your prescriptions from your nominated Pharmacy. However, if you don’t wish to use your nominated Pharmacy for a particular prescription just make sure you let the surgery know when you request the prescription. For example if you wish to collect a prescription from the surgery reception, or a different pharmacy.
- Nomination is very flexible and can be cancelled at any time by asking a member of staff at the pharmacy or the surgery.
Your Accountable GP
NHS England is requiring all GP surgeries to provide all their patients with an ‘Accountable GP’ who will have overall responsibility for the care and support that our surgery provides to them.
Having an ‘Accountable GP’ does NOT prevent you from seeing any GP in the practice as you currently do. You will be aware that none of the GPs are here every single day and patients must not delay medical care by waiting for their ‘Accountable GP’ to become available. As we are a group practice, you may see any of the GPs here, including Locum Doctors and Registrars. Your care will not change.

Please note that our next Staff training afternoon will be on Wednesday 20th September 2017 from 2pm until 4pm and the surgery will be closed. If you need to ring the surgery urgently you will be put through to an out of hours telephone service.

PATIENT RECORDS – DATA EXTRACTION
A new system of data extraction from records in the NHS by the Health & Social Care Information Centre (HSCIC) is to roll this out across the country beginning in Autumn 2013.
This involves patient-identifiable data being extracted from records, linked with other data from hospitals and social care and then cascaded through the NHS, or potentially bought by researchers or private companies for use outside the NHS.
If you wish to Opt out of this process, please inform the practice in writing and a code will be put on your records to prevent any data being extracted to the HSCIC.
Please note that this is separate from the non-Patient- identifiable data which we currently extract for Public Health Protection and Research purposes and will continue to do so.

NHS 111 service
NHS 111 is a new service that has been introduced to make it easier for you to access local NHS healthcare services in England. You can call 111 when you need medical help fast but it’s not a 999 emergency. NHS 111 is a fast and easy way to get the right help, whatever the time.
NHS 111 has replaced the GP Out Of Hours service. If you telephone your doctors Surgery outside of surgery opening hours you will be put through to NHS 111.
NHS 111 is available 24 hours a day, 365 days a year. Calls are free from landlines and mobile phones.
When to use it
You should use the NHS 111 service if you urgently need medical help or advice but it's not a life-threatening situation.
Call 111 if:
- you need medical help fast but it's not a 999 emergency
- you think you need to go to A&E or need another NHS urgent care service
- you don't know who to call or you don't have a GP to call
- you need health information or reassurance about what to do next
For less urgent health needs, contact your GP or local pharmacist in the usual way.
For immediate, life-threatening emergencies, continue to call 999.

Notice For Patients Regarding Research Data Collection – Revised July 2011
London Road Medical practice
The staff at this practice record information about you and your health so that you can receive the right care and treatment. We need to record this information, together with the details of the care you receive, so that it is available each time we see you.
The information recorded about you may be used for reasons other than your personal care, for example, to help to protect the health of the general public, to plan for the future, to train staff and to carry out medical and other health research.
We are involved in research studies which require access to anonymous information from patients’ notes. You cannot be identified from these notes as all personal details (name, address, post code, full date of birth) are removed. Individual patients’ records are added into a much larger anonymous database from many patients across the UK which is used by researchers outside this practice. This data may be anonymously linked to other data, such as hospital data.
If you would like to opt out of this data collection scheme, please let your doctor know and your records will not be collected for use in the anonymous research database. This will not affect your care in any way.
If anything to do with the research would require that you provide additional information about yourself, you will be contacted by your GP to see if you are willing to take part; you will not be identified in any published results.
You have a right of access to your health records. If at any time you would like to know more, or have any concerns about how we use your information, you can speak to the Practice manager on 01452 522079.
